Click on the form you are interested in downloading. Change of Address Form -- All address changes must be reported to the Board in writing. Copy Request -- Use this form to request copies of previously filed Applications and/or Declarations. Credential Evaluation Information for Rule XIII applicants who received their initial law degree outside of the United States. Exemption From Release of Bar Examination Results -- See §82.029 of the Texas Civil Statutes FAST Form -- Fingerprint Application Services of Texas General Waiver Request Form -- For applicants who wish to petition the Board for a waiver of a rule or rules found in the Rules Governing Admission to the Bar of Texas (except Rule XI(f), which requires a special request form below.)Special Request For Waiver Of Five-Time Limitation Form – only for applicants who wish to request a waiver of the five-time limit on taking Texas Bar Exams in Rule XI(f). Do not use this form if you are requesting a waiver of any other provision. Do not submit a request to waive Rule XI(f) before reaching the five-time limit or while you are awaiting grade release for the last exam taken. JD Certification Form -- Graduates of out-of-state law schools, as well as all attorney applicants, must submit this form to their law school. The law school should return the completed form directly to the Board of Law Examiners. If you are a non-attorney graduate of a Texas law school, do not use this form unless you have been specifically instructed to do so by Board staff. No individual will be recommended for licensure to the State Bar of Texas without documentation of his or her J.D. The General Application contains this form. Multistate Bar Exam ("MBE") Score Transfer Request Form -- For individuals who wish for the Board to send verification of a prior MBE score to another jurisdiction. (Texas does not accept MBE scores from other jurisdictions.) Policy Statement on
Practice Requirements -- Attorneys licensed in another state, but without holding a Texas
law license, may use the following activities in Texas for purposes of
meeting the practice requirements of
Rule XIII, Rules Governing Admission to the Bar of Texas.
